New to Area/Practice – How to become a New Patient?

New to the area or transferring from another pediatrician?

Whether you are new to the area or just new to the practice, Cornerstone Pediatrics is thrilled to partner with you to provide your family with leading pediatric care.

We make switching pediatricians simple!!

    • Verify that we are in network with your insurance plan (see Accepted Insurances).
    • “Schedule an Appointment” as a New Patient and complete the appointment information form.
    • Cornerstone’s New Patient Coordinator will reach out via email for next steps and to schedule your first appointment.
    • If you are transferring from out of the country, please send your vaccine records to cp-newpatient@rdmgpa.com or fax to (919)481-3952.
    • If you have specific questions, please feel free to contact us.

This process takes about a week, depending on receiving vaccine / medical records from previous practice.

VACCINATIONS

Cornerstone Pediatric and Adolescent Medicine (CPO) policy requires all patients under the age of 2 to be vaccinated against vaccine-preventable diseases prior to the patient’s 2nd birthday.

CPO’s vaccine policy includes the following vaccines:

  • Hepatitis
  • Tetanus and Pertussis (DTaP)
  • Haemophilus influenzae type b (Hib)
  • Polio
  • Measles, mumps, and rubella (MMR)
  • Varicella
  • Pneumococcal conjugate (PCV)
  • Rotavirus
  • Tdap (at 11 years old)
  • Meningococcal (at 11 years old)

Patents who are over 2 years old, and not vaccinated as required by CPO policy, must be fully vaccinated within 7 months. This grace period allows plenty of time to bring immunizations up to date.

Documentation of all vaccinations must be provided to Cornerstone Pediatric and Adolescent Medicine. Failure to comply with this policy will result in the denial of healthcare services at Cornerstone Pediatric and Adolescent Medicine, as well as possible exclusion from group settings, such as daycare or school, as required by North Carolina law.
Exemptions will only be granted for medical reasons, as determined and/or
agreed to by a Cornerstone Pediatric and Adolescent Medicine physician. Religious and personal belief exemptions will not be granted.
