During a routine well-care visit, the doctor will perform a complete physical examination to monitor growth and development and detect any changes or problems. We will also discuss and administer any immunizations that are due.
Well visits usually involve important screening questionnaires that help your doctor thoroughly and efficiently evaluate development and mental health.
Before your appointment, you will receive a previsit text and/ or email that will contain any paperwork needed for your child’s upcoming visit. We require that these forms are completed prior to seeing your doctor. These forms will include review of demographic, insurance, & family history and many also have screening questions to help our doctors access developmental milestones.
If you have a child under the age of 5, you may want to download the CDC’s Milestone Tracker App to follow your child’s development along with us.
Please write down any questions you have and bring them with you to the appointment. We look forward to discussing them with you!
Sports, Camp, College, and Daycare Physicals
Sports, camp, college, and daycare physicals are performed at the time of your well-care visit. If your child needs a sports, camp, daycare, college, or kindergarten form filled out, please print it at home, fill out your portion, and bring your form with you to your well visit. If you have additional concerns at the time of your appointment, please let your scheduler know so we book enough time for your appointment.
The American Academy of Pediatrics (AAP) recommends routine well-care visits at the following ages:
| 48 to 72 hours of age
