To request an invitation for a child 17 years of age or younger:
Please complete the Follow My Health Patient Portal Invitation Request Form and return it via fax to (919) 481-3952, mail it to our office at 97 Cornerstone Drive, Cary, NC 27519, or hand deliver it to our office. Please note, in order to ensure patient privacy, we do not accept forms via email.
For patients 18 years and older:
Please complete the Follow My Health Patient Portal Invitation Proxy Request Form and return it via fax to (919) 481-3952, mail it to our office at 97 Cornerstone Drive, Cary, NC 27519, or deliver it to our office. Please note: The 18-year-old patient must be the person to fill out the form and sign it.
Please note, in order to ensure patient privacy, we do not accept forms via email. Only faxed, mailed or hand-delivered paper forms can be accepted.
Once the paperwork has been received in our office, it takes about one week for access to be granted. We will send you an email with an invitation. Please access the portal link via a laptop or desktop computer, a mobile device will not work.
Once you gain access to our patient portal, you’ll be able to do the following:

- See upcoming and past appointment dates
- View your child’s recent immunizations and health information
- Send & receive non-urgent secure messages to our triage nurse (allow 3-5 business days)
- Receive important messages from us regarding your child’s health or regarding other important happenings at our clinic
At this time we are unable to receive appointment requests via the portal.
